The biggest issue with Sonic Adventure is the janky cutscenes and despite some nice recreations existing on youtube I've never heard tell of anybody creating a mod to fix or replace them.

Here's one of the main mod authors showing off some of the various differences. Proper gamepad support is great (xinput just works now), really nice resolution and window options. (Except for SADX character models, I love the character models. Throughout the various ports of Sonic Adventure textures got worse, and worse.
